Return and/or Repair Authorization Form

 1) The following form must be completed in order for us to issue a Return/Repair Authorization number. 

2) We will examine each case individually and decide whether the item should be returned to our factory for repair, have the repair done locally, or whether it is most cost effective for us to simply replace it.
 

3) We will email your return/Athorization number and a decision as to what we conclude should be done to fix the problem.

4) If we conclude that the repair must be done in our factory, please ensure that a copy of our emailed reply to you clearly showing the authorization number of the piece to be returned, accompanies the item. 

5) Please keep a copy of the information from this form for your records.

6) We will not accept shipments or be liable for repair bills without first having provided an authorization number.
